To some of the most serious threats of the modern world belong problems associated with lifestyle diseases. The expanding list of population health problems begins to enforce a broader view of the proper functioning of public health in Poland and leads to discussions about building a modern integrated healthcare system. The aim of the paper is to bring doctors closer with principles of coordinated healthcare – working both, in hospitals and in outpatient care.
