Respiratory infections continue to pose a threat to the health and lives of the sick. Lung infections especially atypical etiology require treatment with macrolide. A comparative analysis shows that the best properties are azithromycin with respect to other available macrolides. Both the antimicrobial activity spectrum and the immunomodulatory activity of azithromycin make the indications for its use systematically extended also to the inflammatory processes in Lyme disease.
