Treatment of bone nonunion (pseudarthrosis) is a difficult orthopedic problem. Patients often need reoperations and hardware replacement. The study presents a minimally invasive method of treatment of long bone nonunion using platelet rich plasma (PRP).
The aim of the study
was to compare the effectiveness of platelet rich plasma treatment in patients with hyper- and hypotrophic nonunion.
Material and methods
. The study included 81 patients of both sexes with nonunion after surgical internal fixation. Each of them received a course of 3 PRP injections every 2 months with additional drilling of the fracture site. The treatment result was evaluated based on control X-ray images. 4-6 months after the last procedure.
Results
. We observed bone union in 83.8% of patients with hypertrophic nonunion and 52% of patients with hypotrophic nonunion. Also, we noticed a statistically significant correlation of nicotine abuse with a negative treatment result of nonunion – 40% success rate compared to 75% with non-smoking patients.
Conclusions
. PRP injections are an effective treatment for bone union. Hyperthophic nonunion responds better to PRP injection treatment than hypotrophic nonunion. Nicotinism negatively affects the results of the treatment.
